Gender Affirming “Top” Surgery

“Top” surgery, also known as chest surgery, removes, enhances and/or reshapes the breast tissue in order for patients to achieve a more masculine or feminine look. There are several different approaches to this procedure based on the patient’s desired results and unique needs.

Some mastectomy techniques for transgender men include:

Liposuction is typically combined with other surgical incisions to remove excess tissue. However, for patients with small breasts, small liposuction incisions can be used to remove breast tissue with minimal scarring.

Best for patients with smaller breasts, this incision is made under the areola to remove tissue.

Best for patients with an average or moderate amount of breast tissue, a circular incision is made around the areola to remove tissue and skin.

This incision works best for patients with larger breasts. One incision is made at the top of each breast to remove tissue and skin, then a second incision is made to reposition the nipple.

During this procedure, Dr. Ghaznavi will remove all of the breast tissue, excess skin and nipples. The nipples will then be resized and replaced so that they are proportionate to the new breast size.

This procedure enlarges the breasts using implants. Patients can select a specific implant material, shape, size, surface and projection based on desired results.

Rather than using implants to enlarge breasts, this procedure involves taking fat from an unwanted area of the body and injecting it into the breasts.

Fat transfer, also known as fat grafting, allows Dr. Ghaznavi to take fat from an unwanted area and place it in an area that may feminize the body. Fat can be transferred to the buttocks, also known as a Brazilian butt lift, the hips, the thighs and more.

To give the body a more masculine appearance, fat can be removed via liposuction from more traditionally feminine areas, such as the hips.

Body implants can be used to give the body a more toned and masculine appearance. Implants can be inserted into areas where the patient may want to appear more muscular, such as the biceps, the pectorals and more.

Patients who undergo these procedures must meet both mental and physical health requirements. Most patients who seek gender-confirming care have been diagnosed with gender dysphoria—or the distress caused when one’s gender identity does not match their biological sex characteristics—by a mental health professional. In order to qualify for surgery, patients must have a letter of recommendation from a mental health professional that outlines why these procedures may be beneficial to the patient.
